It seems pretty insane to me that this is in fact not the case (in the US). When the barrier of entry to physically transitioning is so low, it's a no-brainer that some young people are absolutely going to jump in, without first having received the appropirate professional counseling to determine whether they're even trans.

Of course, this has to be balanced out against a system where gatekeepers are denying actual trans people access to medical care for kicks or to uphold the status quo.
